The Elizabeth City State University Office of Emergency Management/Environmental Health & Safety is dedicated to the protection of human life and our campus environment. This office acknowledges the close relationship between the environment, work, and human health and safety.
We are committed to preventing the loss of human potential caused by fatalities, injuries, illness, and disabilities on the job and in the campus community.
How to File a Report
Please review the Incident Investigation and Reporting Program. The employee completes the Employee’s Incident Report Form and submits it to his/her supervisor or to the Department of Environment, Health and Safety.
The supervisor completes the Employer’s Report of Injury to Employee and forwards it to the Department of Environment, Health and Safety within 24 work hours of knowledge of the incident. The supervisor must also complete the Supervisor’s Incident Investigation Form, to be submitted to the Department of Environment, Health and Safety along with Form 19.
University and OSHA Safety Information
All establishments covered by Part 104 must complete the OSHA log summary reports. The University is required to complete these summaries and ensure that they are readily available to the employees. This log is required on an annual basis beginning February 1 until April 30. Below are links to ECSU’s OSHA log summaries.
